One agency should be made responsible for the coordination of all street projects. One agency to control the funds, create the timelines and grant permits to all the utilities.

There's an opportunity here for livability advocates and progressive-leaning neighborhood associations to check the Five Year Paving Plan for streets in their areas and initiate/support traffic calming strategies for them. The best chances for making changes are in Years 03-05 where projects are more likely to not already be fully planned and contracted. All the better to work with DPW and other city staff to implement Complete Streets treatments and incorporate grassroots input. See the Five Year Plan at sfdpw.org and search "five year paving plan."
